TRANSPORTATION
Round trip transportation from the airport and the hotel ONLY if booked through our web site and this concierge service that your healthcare provider has set up for you as a convenience to make your experience seamless. After booking is complete and confirmed, an email will be sent with your hotel confirmation information and at this time, we will also ask for your flight information so we can coordinate with the shuttle service for your airport pick up. ***Please note that if you are arriving on Sunday after 8pm the Henderson Inn & Suites shuttle will not be able to pick you up from the airport because service stops after 8pm. If your flight is coming in after shuttle service is available, we are more than happy to assist with offering discounted transportation alternatives that would billed directly to the patient or contracted company assisting with the scheduled trip.
For BeLiteWeight Insurance patients staying at the Henderson Inn & Suites BeLiteWeight will offer round trip transportation for you and your guest (no minors) to North Vista Hospital for your surgery.
Any additional fees that would potentially be charged in addition to the pre negotiated room rate above would have to be approved by the client prior to the card on file is charged aforesaid additional charges. I hereby authorize the selected hotel charge the credit and/or debit card listed above.

I understand that the hotel requires a deposit in the amount equivalent to one night stay, which is refundable at time of check out using the credit or debit card above. If you need to cancel this reservation, call us a minimum of 72- hours prior to stay. One night charge will be incurred if cancellation is less than 72-hours. Client agrees to indemnify, defend and hold harmless EVENTURE, Blossom Bariatrics and the Hotel Client is reserving rooms, including their officers, independent contractors, employees, agents and heirs and assigns, from any and all liability, costs, damages and attorneys’ fees resulting from or relating to any claims in any way connected with their reservation at the hotel or the transportation provided from the hotel. This indemnity, defense and hold harmless provision extends to any damages caused by the Client or the Client’s guests or attendees in connection with their reservation and the amenities included in the reservation.*
